In other states, sometimes this form is called the Health Care Power of Attorney, which can lead to confusion. In New York, the Health Care Proxy Form is different from the Power of Attorney Form. Here, the Health Care Proxy Form enables you to appoint someone to make medical decisions for you, while the Power of Attorney Form enables you to appoint someone to make financial decisions for you. These forms are not interchangeable – the Health Care Proxy Form does not give your health care agent the power to make financial decisions for you, and vice versa.
