Anyone can make an initial hospice referral. Most commonly, a medical provider determines if the patient will make the initial referral, so we recommend speaking with your medical provider. However, if your medical provider is reluctant or resistant, a patient, caregiver, or loved one can make the initial contact. After the initial referral, the hospice program will confirm your eligibility and discuss next steps.
