Who is part of the palliative care team?

At a minimum, the palliative care team usually includes a doctor, nurse, and social worker. But it can also include many others including physician assistants, nurse practitioners, pharmacists, nutritionists, chaplains (they offer spiritual care), physical therapists, and mental health providers.
