Do I need a lawyer to complete the Health Care Proxy Form?

No lawyer is required to complete the form and it doesn’t even have to be notarized. It only requires that you sign it in front of two adult witnesses (who also must sign it). These witnesses can be any adult, but cannot be the same person you list as your health care agent or alternate agent.
